EDIT : Oh, and on behalf of all people who have to data entry, exactly what is so hard about getting different firms to use one standardised form so the people who have to input the data at the other end can do it quickly and efficiently and not have to search the form high and low for something, or desperately try to interpret a comment. I don't give a fuck, this is not about indiviuality, this is about clear, simple layouts on paper. Not a work of art.
