I've been pretty happy with Purple Paw (http://www.purple-paw.com). On the actual domain-moving, for a .com you tell the new host what your domain is, and might have to authorise the move - if so, they'll let you know the details. It's pretty straightforward.
Um... good. I've never had any problems FTP-ing in, and everything's uploaded nice and fast. Server uptime is 99%+, it seems to be pretty robust, and it comes with a nice version of Control Panel.
I use Adaptive.net, and haven't had any problems. They've been lovely, actually, and there might possibly be a sale going on now. As for decent space, they've got different sized accounts - I don't need a lot, so I went small.
